Page 5 - The applicant or his/her agent must complete and sign this page

Page 6 - The medical practitioner or allied health professional must complete this page on all applications in addition to the section relevant to the nominated criterion.

Page 7 - Criterion 1 - dependence on a wheelchair/scooter for all mobility outside of the home

  • A treating medical practitioner, specialist or allied health professional must complete and sign this criterion.
  • This criterion includes people who are bedfast or chairfast or have limited mobility within the home and require a wheelchair/scooter for all mobility outside the home.
  • Child/infant seated mobility devices (large strollers with special modifications) are equivalent to wheelchairs. Infants and children with disabilities who are prescribed seated mobility devices by the treating medical practitioner or therapist may be eligible for this scheme.

Page 8 - Criterion 2 - significant mobility restrictions

  • A medical practitioner, specialist or allied health professional must complete and sign this criterion.
  • Functional details, including symptoms restricting mobility and type of mobility aid, if used, are required.

Page 8 - Criterion 3 - significant visual impairment in both eyes

  • An Ophthalmologist or Low Vision Coordinator/Assessor must fully complete and sign this criterion.
  • Criterion 3 relates to significant visual disability affecting both eyes.
  • Applicants must have significant visual impairment entitling them to the Aged Pension or Disability Support Pension (blind) or a visual disability within this range of severity, such as visual acuity (best corrected) 6/60 or less each eye and/or significantly restricted visual fields.

Page 8 - Criterion 4 - severe and uncontrollable epilepsy, with seizures involving loss of consciousness

  • A treating medical practitioner or specialist must complete and sign this criterion.
  • Description and frequency of seizures that involve loss of consciousness must be provided.
  • Temporary membership only is available for applicants within this criterion
  • Applicants with epilepsy who are in a stable condition as a result of medication are not eligible for this scheme.

Page 9 - Criterion 5 - significant intellectual disability, memory or communication impairment

  • A treating medical practitioner, specialist or allied health professional must complete and sign this criterion.
  • This criterion applies to applicants with moderate to severe intellectual impairment and also applies to applicants with moderate to severe dementia.
  • The ability to manage activities of daily living, behavioral problems and safety issues are considered in assessing applications within this criterion.
  • The applicant must require the assistance of another person at all times for travel on public transport.

Page 9 - Criterion 6 - significant psychiatric disability

  • This criterion applies to significant psychiatric disorders with gross disorganisation in relation to day-to-day activities.
  • A Psychiatrist, Psychologist or treating medical practitioner must complete and sign applications in this category.
  • The applicant must require the assistance of another person at all times for travel on public transport.

Appeals

  • All appeals must be in writing and provide additional information from a medical practitioner or other health professional to support the claim for eligibility within one of the six criteria of the scheme.
  • The appeal panel might seek further information if required.

Privacy
The Department of Planning and Infrastructure collects the information on the Northern Territory Taxi Subsidy (NTTSS) application to enable assessors to assess the eligibility of the applicant for NTTSS membership.

The Department of Planning and Infrastructure will not disclose any individuals details to other third parties without their consent or unless required by law.
